Coordinates of collision: 3°46'56.11"N, 108°17'35.03"E
On the 14th of May 2010, AIR LABUBLOX Flight 2261, a Roeing 737-600 passenger jet, and PK-IFS, a McBentley Bouglas BC-10-30 cargo jet collided in mid-air over Natuna Regency, a regency in the Riau Islands. All of the passengers and crew aboard both planes on the day were killed, resulting in a total death toll of 76. The accident would be known as the 14-14 Tragedy.
The accident was investigated by the Bloxdonesian National Transportation Safety Committee (NTSC), when they discovered that the BC-10's transponders were broken. Nobody on the plane was informed about this, and as such the 737 was unable to detect the airplane.
The Perbadanan Penerbangan Negara (PPN), the body responsible for the operation of Labubloxian aircraft, issued a public apology the following day, stating that the pilot operating the Air Labublox plane didn't have a legal license. This problem would become even worse as the years go by.

Roeing aircraft and crew
The Roeing 737-686, registered as LB-S7196 (First flew and Delivered to AIR LABUBLOX in 2003), was operating Flight 2261 from Bricktoria International Airport to Penang International Airport then Dhaka with 68 passengers and 4 crew. The flight departed Bricktoria Airport at 12:57. Flight 2261 was commanded by 39-year old Captain Bakar bin Abdullah. The F.O was Mohammad Ali bin Harraz, both of whom died during the accident.

McBentley Bouglas aircraft and crew
The McDonnell Douglas DC-10-40, registered as PK-IFS (First flew in 1979, and conv rted and delivered to Voodoo Cargo in 1996), was doing a chartered cargo flight from Sultan Hasanuddin International Airport (Makassar) to Don Mueang International Airport then Thomas Anderson International airport with 4 crew members. The plane departed from Makassar at 13:02. PK-IFS was commanded by 41-year old Captain Samsul Sutrisno Andi with F.O Saiful Tatang Amir and Flight Engineer Deni Adi Nurulputra. The flight was also accompanied by Loadmaster Andrew Davis. The plane had no flight number and was instead using it's registration.

Accident
(14:06) While PK-IFS (BC-10) was going straight, the transponders were unknowingly broken prior to the flight, so they kept a straight line. Both Air Labublox 2261 and PK-IFS had recieved TCAS warnings. PK-IFS received a descend call (descend, descend now!) while Air Labublox 2261 recieved a climb call (climb, climb now!). AIR LABUBLOX 2261 (737) was flying at the same level PK-IFS was, due to the fact they never knew that they were in each other's paths. This wasn't until 8 minutes later (14:14) that they were close enough to notice one another. PK-IFS had been descending but at a very slow and gradual rate. Meanwhile, Air Labublox 2261 had misread the TCAS call as “descend”. At 14:15, as soon as the “increase climb, increase climb” call came to Air Labublox 2261, they initiated a climb correcting their mistake, but it was too late. At 14:15:22, The BC-10 crew assumed that the 737 avoided them until they heard a massive, powerful "bang" noise. The 737 had attempted to escape the path of the BC-10, but because their speeds were equal, this didn't help and the passenger plane had yawed too late, and the banking also did not save them. The pilots were unsure if they will hit the wing by banking to the right, but banked to the right anyway. The 737's cockpit section struck the tail engine which broke the cockpit windows sucking the pilots into the engine killing them, while the right wing hit the BC-10's right elevator. The entire empennage assembly of the BC-10 was ripped, causing massive instability to the BC-10 causing the plane to drop whilst cargo was being dropped out of the tail causing instability, and eventually the cargo plane crashed into Natuna-besar island after a brutally quick nosedive. The 737 however, broke into several pieces. Eventually all parts of the plane had fallen into the island. Eyewitnesses had seen falling metal parts. The site of BC-10's debris was set on fire after a spark, with no survivors in sight.
Aftermath
Bodies
Only 5 bodies were recovered from the accident, 1 was a crew member who was believed to be Andrew Davis from PK-IFS and 4 were passengers from 2261. It was presumed that everybody involved in the accident had died, due to the fact that not enough bodies were found. To this day, the rest of the victims are a mystery.
Memorials
Flight 2261
The original memorial was erected for AIR LABUBLOX 2261. It was a black granite block with golden text showing the names of all occupants onboard, and a farewell message to everyone onboard. It was eventually gone and stolen, believed to be done by North Neo-Korbloxians with intentions to dishonor & disrespect the tragedy. It was soon discovered that it was destroyed into multiple fragments spread all over Bloxraman Island. Eventually a new (and the current) memorial was erected as a huge metal sign with embossed words with the same message and victims.
PK-IFS
The original and current memorial is a simple metal (hollow inside) pyramid made from the fuselage material of the BC-10 involved in the accident, with the names of each crew member from shortest to longest with mentions of it being involved with flight 2261.
Investigation
The Bloxdonesian National Transportation Safety Committee (NTSC) released its report on the accident on 15 May 2010. The determined cause of the crash was believed to be broken transponders on the BC-10-30F PK-IFS suggested from an inspection document wherein Voodoo serious safety issues beforehand. The NTSC grounded all of Voodoo's BC-10s until all transponders were fixed. Another set of investigations were carried out the same day, checking the flight log. They discovered that the headings were intersecting each other, suspectfully believing that it caused the crash, and they discovered that the Labubloxian pilots and crew members had no legal license.
This functioned as a wake-up call for the Perbadanan Penerbangan Negara to address this issue of illegal licences, however they did not do anything for more than a decade and a half until they finally solved the issue in early 2026. Only two crew members were caught by the MODKEB. As a result of this incident, the NTSC temporarily suspended flights to Labublox for all Bloxdonesian airlines, and demanded that the Perbadanan Penerbangan Negara do the same for all Labubloxian airlines until the issue is solved.
From 2010 to 2025, the convicted crew members and the co-pilots (there were two) were gradually arrested by the MODKEB. The saga finally ended when the MODKEB finally caught several Labubloxian civil servants who helped to cover up Captain Bakar's illegal licence by making false statements and offering bribes. They were sentenced to life imprisonment after being charged with medium-level treason.
