On October 5, 2000, an incident occurred, that involved a Roing 747-400 and a RRJ-145.

The accident happened when the 747 was taking off from runway 70L and the RRJ 145 landed on Runway 1L. The crash was caused when the ATC was abandoned, causing Pilots to think that the ATC Allowed them for Clearance.

The survivors were Wes Rowingson, Andy Ganrerds and Panede Jaril. The witnesses were photographer Landers Kingston, a person at Terminal 1A and a ground crew personal at Gate 6P.

Investigation

RRJ-145 (Scoot-8)

RTSB have investigated the plane and saw that the controls connected to the elevators have broken while on touchdown, which caused the plane to be unable to be pulled up. The rudder might have been damaged due to this.

Roing 747-400 (Neo-Japan Fight 923)

RTSB have investigated the plane and saw that the pilots never had any experience operating a 747 prior to boarding this aircraft; they only had experience on small planes or medium planes. The pilots were told that they were delayed because of damages in the engine caused by the reverse thrust failing to stop the plane completely, so when they arrived at the airport or a reject takeoff they use the brakes or the parking brakes.

Timeline of events

8.30 P.M - 9.00 P.M

ATC in Perth cleared Neo-Japan Airlines Flight 923 to taxi to Runway 7L while a Scoot Airlines plane is coming to the second runway.

Scoot Airlines Flight 8 was on approach to Runway 1L with ILS on. Co-pilot Algren said that there was another plane at Runway 7L, but Captain James Oliver said that the system was bugged and Runway 7L appeared to be occupied by a plane even when there was no plane.

9.20 P.M - 9.30 P.M

Flight 923 was cleared for takeoff and ATC went on break. Flight 8 already touched down on Runway 1L.

Flight 923 sees flight 8 and tries to cancel the takeoff procedure, but reducing the thrust failed, causing the plane to not stop as the pilot didn't took the Warning made by the Ground Crew too Seriously

9.40 P.M - 10 P.M

Flight 8 have aborted the landing but then failed to takeoff again, colliding with the 747 upper deck and the fuel tank. Some of the passengers on Flight 8 have survived and were evacuated to a nearby grass field.

The black boxes was found at the front of the plane, but then was found at the cabin. It was revealed the plane was not able to pull back up.

Neo-Japan Flight 923

The CVR was destroyed when the plane touched the upper deck. However, the Black Box has shown that engine one and four had failed to reverse the thrust.
