On June 19th, 2031, a major lab accident has occurred, claiming the life of a single personnel member and no secondary infections were recorded. This was one of major accidents in a high-safety facility.
Prelude
At around 5:38AM (0538), The Fortress manage to finalize all Tan Industries modified E-Virus samples into bio-storage. Finalization report was made and archived by the lead project manager, Doctor Phillip Reed.
Twenty-three minutes later, at 6:01AM (0601): Professor Michael S. Derick and Stella Wong clock in for work with the day shift employees to replace night shift employees. Secretly, Stella was an insider planning a heist to steal the TI-mE-Virus samples so that it could be given to Monika Corporation.
Then later at 9:53AM (0953), Dr. Reed gathers Prof. Derick and Stella Wong for the planning of the analysation of the modified E-Virus for it's effect on wildlife, which at 10:21AM (1021); they gather at Special Biological Laboratory 2 to conduct the test.
The incident
At 10:25AM (1025), Stella immediately held Dr. Reed and Prof. Derick at gunpoint with a 3D printed Glock 18, before demanding for them to give the modified E-Virus samples. Dr. Reed immediately insulted Stella asking if she "had lost her mind", which resulted in Stella immediately threatening to shoot them. Prof. Derick immediately gave a bio-container containing the sample, which Stella incapacitate Dr. Reed and attempt to incapacitate Prof. Derick, but immediately was disarmed and forced into a struggle lasting for 10 seconds, which resulted in Prof. Derick being incapacitated, though he manage to hit the security alert button to alert security; forcing Stella to flee.

Dr. Reed: "Remarkable, the sample affects animal specimens similarly, but would die off shortly after-"
*Stella immediately grabs a matte painted Glock 18 under her lab coat, and immediately pointed at Dr. Reed and Prof. Derick*
Stella: "Twitch and your dead."
Stella: "Give me the sample bio-container."
Dr. Reed: "Stella, have you fucking lost your mind?"
Stella: "Shut up or I'll shoot. Now give it."
Dr. Reed: "What your doing is treason! You'll be dead before you flee!"
Stella: "Well I'll be shooting you guys before I flee."
Prof. Derick: "...Fine, just don't do it. We were just doing work."
*Derick hesitantly but compliantly gives the bio-container*
*Stella immediately incapacitate Dr. Reed, but Prof. Derick manage to disarm her and attempt to attack her. But she manage to knock Derick out, which Derick manage to activate a Security Alert Button. Which Stella immediately ran out of Special Bio-Lab 2.*
At 10:27AM (1027), T-Security Agent Ritterman and Agent Wesker, were immediately enroute to respond to the security alert. Which they found only the unconscious bodies of Dr. Reed and Prof. Derick. Five minutes later, after they analysed the lab, they found out a bio-container was missing and immediately contacted central command to enact a full lockdown.

*Agent Ritterman and Agent Wesker arrived to Special Biological Laboratory #2 from Biology Hallway Sector 1A*
Ritterman: "Central Command, this is Agent Ritterman. Clearance ID 4856-8. We have arrived at the location of the Security Code Blue Alert."
*Wesker checks on Dr. Reed and Prof. Derick, they were simply unconscious*
Wesker: "Unconscious but breathing... Get Medical to here."
*Ritterman noticed a bio-container missing*
Ritterman: "Wesker... one of the container is gone."
Wesker: "Call it."
Ritterman: "Central Command, this is Ritterman. We got a bio-container missing at Special Bio-Lab #2. Initiate Class-2 Lockdown."
At 10:31AM (1031), all T-Security Agents were mustered up and prepared to search for the missing bio-container and to evacuate Tan Industries personnel to ensure no one is affected.
As of 11:59AM (1159), all Tan Industries personnel (including Reed and Derick) were searched and evacuated to safety outside of The Fortress at the surface floors. T-Security later only unaccounted Stella and immediately speculate that she is the personnel who stolen the bio-container and immediately started an armed search around the facility in an attempt to apprehend her. Accounting that the facility is extensive with at least 50 floors underground they accounted that it may take at least two to five hours to search the entire perimeter.

*All Tan Industries personnel were evacuated to Gate B where they were at a designated emergency shed to await for the all clear announcement*
T-Security Commander: "ALL T-SECURITY AGENTS, MUSTER UP!"
*All T-Security agents were mustered up for the briefing."
T-Security Commander: "Good day agents! As you may have known: recently a lockdown alert was initiated and personnel was evacuated. As of the lockdown evacuation. All 3,460 facility personnel, including Site Director, were accounted for. Currently, Assistant Researcher Stella Wong is missing and is speculated to be responsible for the theft."
T-Security Commander: "Due to this revelation, the Site Director authorized for Procedure E-5 Armed Search. As such, you are authorized to utilize lethal force in an attempt to stop the perpetrator. Only shoot to kill when the perpetrator poses harm. Do you understand?"
T-Security Agents: "SIR YES SIR!"
T-Security Commander: "Arm up and prepare for deployment!"
At 2:46PM (1446), T-Security Agent Paz alongside an unnamed T-Security Agent encounters Stella at one of the elevator landing points in an attempt to call an elevator to the surface zone, though due to lockdown protocols, all elevators were locked down and immediately shut down to ensure safety of the surface area. Paz and the unnamed agent demanded for Stella to surrender and slowly place the container down. As a desperate attempt to flee, she charged pass them and immediately dashed into Hallway Sector 4C. Concerned that Stella maybe armed and was in an attempt to attack them, Paz fired a shot from his Glock 34 intended to aim at Stella's leg; but the bullet striked the metal support of the bio-container and then strike Stella. Despite this, Stella still manage to flee. Afraid that the shot may had damaged the bio-container in some way, the unnamed T-Security agent immediately alerted the other security personnel to evacuate the facility immediately due to contaminants; resulting in all T-Security agents (including Paz, Ritterman, and Wesker) evacuating to surface areas through emergency staircases. Following the evacuation, the on-site director was asked to enact a "A3 decontamination" of the facility. T-Security personnel are immediately mustered up again for an Armed Search Protocol D-3 briefing and immediately required to don combat-hazmat suits before re-entry to the facility.

*Stella was trying to reach to an elevator in an attempt to escape to the surface sector, but the lifts were under lockdown due to the lockdown.*
Stella: "Come on... Damn it! F***!"
*Agent Paz (armed with a Glock 34) and an unnamed agent (armed with a MP7) arrived, which they encounter Stella*
Agent Paz: "Stella Wong! You are under arrest for the theft of a biological object! Place the container down slowly and put your hands up!"
Unnamed Agent: "Follow the Agent's order Stella! We are authorized to use lethal force when necessary. Drop the container and surrender!"
Stella: "Shut up! You guys know that if you shoot the container I'll bring you down with me!"
Unnamed Agent: "It doesn't have to be like this! Just surrender yourself!"
Stella: "NO!"
*Stella shoves her way through Agent Paz and the Unnamed Agent, as she went past them and enters Hallway Sector 4C*
*Agent Paz fires a shot from his Glock 34 intending to aim for Stella in the leg, but the bullet ricochet from the bio-container's metal frame and hit Stella in the leg, as she stumbles further into another hallway sector*
Unnamed Agent: "I heard metal striking, you think it hit her or the bio-container?"
Agent Paz: "Likely the container... we need to get out. Alert them!"
Unnamed Agent: "All search units! Evacuate the facility immediately, we got contaminants! I repeat, we got contaminants within the facility! Evacuate immediately!"

*All T-Security agents were evacuated*
T-Security Commander: "Site Director we need to decontaminate the facility. Decontamination Protocol A3."
Site Director: "A3 Decontamination is activated."
T-Security Commander: "Good."
T-Security Commander: "ALL T-SECURITY AGENTS, MUSTER UP!"
*All T-Security agents were mustered up for the briefing."
T-Security Commander: "As you may had heard, a contamination of the facility was detected. As a result, a D-3 Armed Search Protocol is initiated. So before entry for armed search, all agents are required to wear Class-A combat hazmat suits to avoid contact with the Hydro-Peroxide or any secondary infections if the A3 Procedure wasn't effective at decontamination. Is that clear?"
T-Security Agents: "SIR YES SIR!"
T-Security Commander: "Until 1500 hours, where the estimated decontamination procedure will be concluded. All agents are allowed to be on break. Dismissed."
At 3:00PM (1500), the A3 Decontamination procedures was finished and T-Security agents, wearing Class-A combat hazmat suits, re-enters the facility to conduct another armed search for Stella. However; at 3:31PM (1531), Agent Ritterman alongside Agent Paz found the severely disfigured body of Stella Wong, located at joint-Office Hallway 4D. Analysis of the location shows no sign of secondary infections.

*Agent Ritterman and Agent Paz enters Sector 4D in Class-A combat hazmat suits, both armed with a MP7, searching for Stella*
Agent Ritterman: "Control, we have entered Sector 4D at the Joint-Office Hallway."
Central Command: "Roger, we have detected you through CCTV and Motion Sensors. Stay sharp, she may had been infected and may had survived."
Agent Paz: "Command, what was in that container?"
Central Command: "Unsure... We currently don't have access to the storage logs."
Agent Ritterman: "Damn, roger that."
*Ritterman and Paz arrived at a shared conference room near Sector 4D, which they find a heavily disfigured body of Stella Wong, laying slump near the entry to the Shared Conference Room.*
Agent Ritterman: "Holy sh*t"
Agent Paz: "Command, we found Stella... Severely disfigured, not even alive or breathing. She was likely dead during the A3 Decontamination, not caused by the acid... but from an infection the A3 already irradicated"
Central Command: "Did you find the bio-container?"
*Agent Ritterman notice the damaged bio-container, but it's contents was already dead*
Agent Ritterman: "We did, it has a single bullet hole. But it's content is dead, likely caused by A3 Decontamination."
Central Command: "Are you saying that there is no secondary infections so far?"
Agent Ritterman: "Positively. The site is secure, lift lockdown."
Central Command: "Copy that. Containment Successful."
Investigation
After the conclusion of the incident; Stella's body was sent for autopsy and an immediate investigation was conducted. According to the Site Director, the Glock 18, which Stella used to intimidate Dr. Reed and Prof. Derick, was a 3D-printed fake. During internal investigations against Stella, investigators searched her office to reveal a burner phone, and a set of documents, revealing that she was secretly in contact with Monika Corporation and was planning to steal samples of the virus to give it to Monika Corporation. As of now, the incident is classified as both, a security incident, and a lab accident.
Autopsy
The body of Stella was sent for autopsy which the pathologist reveal her cause of death was exposure to the modified E-Virus samples when the bio-container was breached. According to the pathologist, Dr. Harlan Veiss, the body was severely degraded likely due to either the exposure to A3-concentrated hyro-peroxide or the E-virus itself. After the autopsy was completed, her body was incinerated to ensure it does not pose an infection risk.

Dr. Veiss: “Autopsy Report for a confirmed body of Ms. Stella Wong. Identity match verified via dental and retinal scan.”
Forensic Technician Stenford: “External trauma present, but inconsistent with full ballistic lethality alone.”
Dr. Havel: “Note that. Document as secondary cause pending internal review.”
Dr. Haval: "Let's take a peek."
*Assistant Pathologist unzips the body bag containing the body of Stella*
Biohazard Specialist Dr. Marvin Edwin: “Full-spectrum scan is negative. No active viral signatures detected.”
Technician Stenford: “That includes the TI-mE-Virus strain markers?”
Dr. Edwin: “All of them. The system reads as biologically inert.”
Dr. Havel: “Confirm again. This is a modified strain—burnout after host death is expected, but I want certainty.”
Dr. Edwin: “Second scan confirms: no replication activity, no residual transmission vector.”
*Then then analyse the bullet wound*
Technician Stenford: “Penetration wound originates from lateral ricochet trajectory. Impact point suggests deflection off a dense composite object.”
Dr. Havel: “Container casing?”
Technician Stenford: “Most likely. Impact was not a direct shot.”
Dr. Edwin: “That aligns with field report—bullet struck containment unit before entering tissue.”
*Dr. Haval then initialize a full-biological scan of the body*
Dr. Havel: “Any indication of pre-mortem infection progression?”
Dr. Edwin: “Yes. Localized cellular degradation is present—but only post-impact.”
Technician Stenford: “Meaning?”
Dr. Edwin: “Self-contained collapse. Infection initiated internally after physical breach stress, not transmitted externally.”
Dr. Havel: “So no airborne release?”
Dr. Edwin: “No release. No spread. No secondary vector formation.”
*Scans complete and Dr. Haval then takes a look at the scan report, it's clean*
Dr. Havel: “Summarize.”
Technician Stenford: “Cause of death: combined ballistic trauma and acute systemic failure induced by experimental E-Virus activation post-injury.”
Dr. Edwin: “Contamination status: self-terminated strain. Non-persistent outside host.”
Dr. Havel: “Any risk to personnel during retrieval?”
Dr. Edwin: “None detected. All exposure models return negative.”
Dr. Havel: “Then this is classified as containment-successful incident.”
Technician Stenford: “Despite theft attempt?”
Dr. Havel: “The sample never propagated. That is what matters to protocol.”
Dr. Edwin: “Shall we label it a biohazard fatality or security neutralization?”
Dr. Havel: “Both. Dual classification: Security breach and isolated lab incident.”
